ABSTRACT

Objective@#Remuzzi scoring system is utilized for assessing the degree of renal tissue damage in donors with hypertensive cerebral hemorrhage and donors with brain trauma after cardiac death. To explore the prognosis of hypertensive cerebral hemorrhage donor kidney in renal transplant recipients.@*Methods@#The kidney donated by DCD between January 1, 2016 to June 1, 2018 were retrospectively reviewed. Pathological biopsy was performed before transplantation and hematoxylin-eosin (HE) staining after sectioning. The degree of renal tissue lesions was evaluated by Remuzzi scoring system. According to the source of donor kidney, they were divided into two groups of donors with heart failure due to hypertensive cerebral hemorrhage (HCH) and those with brain trauma (BT). Both groups of donor kidneys were preserved by low-temperature machine perfusion. The immunosuppressive regimen was identical in both groups. The prognosis of two groups was compared by serum creatinine (Scr) at Month 1/6/12 post-operation and cumulative graft survival rate over a follow-up period of 12-36 months.@*Results@#The renal Remuzzi score of HCH donors was significantly higher than that of BT donors. The maximal creatinine clearance rate was significantly lower than that of BT donors [(86.8±27.8) vs (115.4±23.2) ml/min, P<0.05]. At 1/6/12 months post-transplantation, serum creatinine levels were (76.1±18.5), (72.4±16.2) and (71.4±16.8) μmol/L in BT group and (160.3±33.4), (154.3±32.6) and (146.4±29.1) μmol/L in HCH group. The SCr in BT group at 1/6/12 months was lower than that in HCH group (P<0.05). Kaplan-meier analysis showed no significant inter-group difference in graft survival between two groups over a follow-up period of 12 to 36 months (Log-Rank test, P=0.485).@*Conclusions@#No significant difference exists in short-term survival rate of kidneys from HCH and BT donors. The recipients of HCH donor's kidney have higher serum creatinine levels than those of BT donors. Selective use of kidney transplants in patients with cardiac death caused by HCH may greatly reduce the waste of donor kidney and improve the quality-of-life of patients with end-stage renal disease.

ABSTRACT

Objective@#To analyze the radiological characteristics of the condylar bone in patients with anterior disc displacement without reduction of temporomandibular joint using cone-beam CT (CBCT), and to further investigate the clinical factors possibly associated with the early-stage osteoarthritic changes.@*Methods@#Two hundred and four individuals (≤30 years old) diagnosed as unilateral anterior disc displacement without reduction (disease duration≤1 year) were recruited. CBCT images of bilateral condyles were independently evaluated by two assessors and documented. Correlation between the early-stage osteoarthritic (OA) changes and gender, age, disease duration and mouth opening were analyzed using SPSS 21.0 software.@*Results@#About sixty percent were presented with OA changes in the symptomatic joints, and most of them (47.1%) (96/204) were early-stage OA changes characterized by loss of continuity of articular cortex (Ⅰ) and/or surface erosion (Ⅱ). Logistics regression analyses indicated that disease duration (P =0.000) and mouth opening (P =0.002) were correlated significantly with early-stage OA changes.@*Conclusions@#Disc displacement without reduction was closely related to OA occurrence. OA changes occurred in more than half of the patients after one month of the joint locked and more possibly in patients with increased mouth opening.

ABSTRACT

Objective To measure the intra-brace pressure in a new fabricated three-dimensional dynamic brace and the intra-cast pressure in the rigid plaster cast,and compare the differences of pressures between the brace and cast. Methods Twenty healthy adult female volunteers involved in this study,with the mean age of 38.4 years.ranged from 26 to 44 years.The forearms of every subject were applied a traditional Dlaster cast and a new fabricated forearm three-dimensional dynamic brace respectively to immobilize the forearm and hand with the wrist at a neutral position,and the applying order was randomly.The intra-brace and intra-cast pressures were recorded at 8 points by using the Fuji pressure sensitive film with the hand at a position of fist and that of extended fingers.The 8 points were the bakc of hand,styloid process of ulna,dorsum of distal radius,abdomen of middle forearm,palm,metacarpus of distal radius,flexion muscle belly,and extension muscle belly.The pressures were compared between intra-brace and intracast.Results The dynamic brace provided greater holding force at the hand and at the distal orearm than the Dlaster cast,and greater immobilization at the wrist while allowing full finger function and greater support to the forearm during the hand movements.Conclusion The results indicates that the new fabricated dynamic brace has measurableadvantages in the immobilization compared to the plaster cast,and especially it can Drovide a persistent traction between the hand and forearm which is important for preventing displacement of the fractured segments.The uew dynamic brace can provide a new technique and represent a further development in the conservative management.

ABSTRACT

Nonunion remains a major complication after skeletal trauma. In the last decade, extracorporeal shock wave therapy has become a common tool for the treatment of delayed unions or non-unions. With the help of a review of the literature, the current author gave an overview of indications, choices of devices, success rates and complications for ESWT in the treatment of non-unions. The conceivable mechanism was also outlined.

ABSTRACT

Objective:To detect the oipA gene of Helicobacter pylori(Hp)strains NCTC11637 and Hp1,Hp2 isolated from clinical biopsies,analyze their nucleotide sequences and make a homologous comparison of nucleotide with Hp 26695.Methods:The oipA gene was detected with PCR in Helicobacter pylori(Hp)strains NCTC11637 and Hp1,Hp2 isolated from clinical gastric biopsies after routine culture.Then PCR products were sent out for nucleotide sequence analysis and compared with Hp 26695.Results:The sequence of the aim gene was obtained in NCTC11637 and Hp1,Hp2 and was made a homologous comparison of nucleotide with 26695.The number of mutation of NCTC11637,Hp1 and Hp2 and was 48,48,50 respectively.The identity was 94%,94% and 94% respectively,while the strain Hp1 was most identical to 11637 as much as 100%.The homology of Hp2 and 11637 was 97%.Conclusion:Hp1,Hp2 and NCTC11637 expresse gene oipA,but the sequences of gene oipA of different strains are distinct.
